Procedeu-se a uma descrição sumária de todos os procedimentos e intervenções que foram necessários para preparar a resposta do Serviço de Doenças Infeciosas do Centro Hospitalar e Universitário de Coimbra à pandemia de COVID-19, na fase de contenção. A estrutura física do Serviço foi totalmente reajustada e as equipas tiveram de alterar profundamente os seus processos. Atualmente, em fase de mitigação, a resposta do nosso Centro Hospitalar foi centralizada no Hospital Geral, que é um hospital dedicado aos doentes COVID-19.

A summary description of all actions and interventions that were necessary to prepare the response of the Infectious Diseases Service of the Centro Hospitalar e Universitário de Coimbra to the COVID-19 pandemic, in the containment phase, was carried out. The physical structure of the Service was completely readjusted, and the teams had to change their procedures profoundly. Currently, in the mitigation phase, the response of our Hospital was centralized at the Hospital Geral, which is a hospital dedicated to COVID-19 patients.

A recente pandemia provocada pelo coronavírus COVID-19 tem tido repercussões mundiais a todos os níveis desde a saude a economia. Os Estados Unidos da America têm sido seriamente afectados, embora de uma maneira heterogénea. Isto deve-se a uma reacçao tardia, uma má coordenação entre os estados, na ausência de uma política federal, um a politização (estamos em ano eleitoral) das medidas necessárias, falta de preparação para este tipo de epidemia, e o advento de informação destorcida. Para isso também contribuiu muito uma pioria da saúde dos americanos, com elevada taxa de obesidade, diabetes, hipertensão entre outras. Vale-nos a colaboração científica que transcende fronteiras, clínicos perspicazes, e uma luta dos elementos de saude publica por uma veracidade de notícias, embora a media seja de culpar, em parte, pelo histerismo causado por notícias incessantes sobre a morbilidade e mortandade deste vírus. Vamos ter de aproveitar para descobrir um melhor modo de viver com este vírus enquanto não houver imunidade de manada ou vacinação eficaz e universal.

The recent pandemic caused by COVID-19 has had major worldwide repercussions across all levels, in particular in health care and the economy. The US has been has been severely affected and remains the country with the highest recorded number of infections and deaths due to this corona virus. The heterogeneous distribution of cases appears to due to a number of factors: individualized responses across different states, a lack of a coherent federal policy, a lack of preparation, the climate of mis/disinformation and the politicization as a result of it occurring in an election year. Add to that a global worsening of health measures for many Americans including obesity, diabetes, hypertension, smoking among others. On the positive side, the scientific collaboration, the observations of astute clinicians and the continued efforts of the public health sector to re-direct the conversation towards scientific truths have played a good part in the fight against this infection. While there is a concern for the hype associated with incessant reporting of the news, let us try to find a common and safe path to live with this virus while protecting the older population and those at risk. Achieving herd immunity or effective vaccination will go a long way towards a return to a new kind of normality.

Immunology of Coronavirus-19 Disease (COVID-19): A Perspective for the Clinician in the First 4 Months of the Emergence of SARS-CoV-2

Nos quatro meses desde a emergência duma infeção pandémica por uma nova estirpe altamente patogénica de coronavírus beta em humanos, a COVID-19, assistimos a uma rápida partilha nas plataformas digitais das mais diversas publicações científicas, numa experiência que se vive também pela primeira vez na comunidade internacional. Neste trabalho, fazemos uma revisão descritiva das mais recentes informações sobre a imunobiologia do SARS-CoV-2, focando as hipóteses que suportam uma deficiência nos mecanismos da imunidade inata e adquirida ao vírus, e o contributo de uma resposta imunitária disfuncional na progressão e mortalidade associadas à doença. Com base nessa nova informação, apontamos as principais estratégias imunológicas nas terapêuticas dirigidas ao vírus, à resposta imune disfuncional e à profilaxia da infeção (imunização). O aprofundar do conhecimento sobre as interações patogénio-hospedeiro, abrirá o caminho para identificar as melhores estratégias para o diagnóstico, terapêutica e controlo da COVID-19, bem como para a sua prevenção.

In the last four months, since the emergence of a pandemic infection by a new highly pathogenic strain of beta coronavirus in humans, COVID-19, we have witnessed the fast sharing on digital platforms of the most diverse scientific publications, an experience also lived for the first time by the international medical community. In this review of the latest information on SARS-COV-2 immunobiology, we focused on the hypotheses that support a defective innate and acquired immunity to the virus, and the contribution of a dysfunctional immune response for the progression and mortality associated with the disease. Based on this new information, we point out the main immunotherapy strategies targeting the virus, the dysfunctional immune response and the disease prophylaxis (immunization). Improving our knowledge on the pathogen-host interactions will open the way to identify the best strategies for the diagnosis, treatment and control of COVID-19, as well as to the best preventive measures

SARS-CoV-2 Immune Response: An Overview

Sobre a pandemia provocada pelo SARS-CoV-2, muito conhecimento tem sido produzido e dado a conhecer, mas muitas dúvidas permanecem por esclarecer acerca da sua interação com o ser humano, nomeadamente sobre se a resposta imune contra este novo coronavírus humano é protetora a longo prazo e qual a possibilidade de reinfeção. A COVID-19 pode assumir diferentes espetros clínicos, desde infeções assintomáticas até formas graves da doença, que resultam da inter-relação entre a patogenicidade própria do vírus e a resposta imune do hospedeiro. Esta revisão foca-se na imunologia da infeção, reunindo não só o conhecimento acumulado com as epidemias associadas ao SARS-CoV e MERS-CoV, mas também o que já se sabe sobre o SARS-CoV-2. Este conhecimento é fundamental e tem implicações quer para o controlo e prevenção da doença quer para o seu diagnóstico e tratamento.

Despite all that is recognized so far on the ongoing SARS-CoV-2 pandemic, many questions remain to be answered, particularly in what concerns pathogen interaction with the human host and whether or not the acquired immune response is protective against reinfection in the long-term. The spectrum of COVID-19 ranges from asymptomatic infections to progressive critical states, a direct upshot from the complex interplay between virus pathogenicity and host antiviral defense. This review focuses on the immunologic response and disarrangements underlying the SARS-CoV-2 pathophysiology, gathering the information not only from the lessons learnt from SARS and MERS-CoV epidemics but also from what is already known about SARS-CoV-2 to date. This knowledge is paramount from the standpoint of disease prevention and control as well as diagnostic pipeline and treatment.

An Internist and Intensivist Reflections in Times of SARS CoV-2 Pandemic

A pandemia pelo SARS CoV-2 (systemic acute respiratory syndrome - Coronavírus 2) espalhou-se por todo o mundo a uma velocidade inusitada. Em vários países provocou infeções graves e simultâneas em tão elevado número que levou à rotura dos sistemas de saúde. Há mais dúvidas do que certezas sobre a fisiopatologia da doença e do seu tratamento, mas já é claro que é uma doença sistémica, que se inicia mais frequentemente por sintomas respiratórios. Não tem tratamento etiológico conhecido e o tratamento de suporte nos casos mais graves é menos eficaz do que o esperado, a requerer estratégias diferentes do que as conhecíamos até aqui. As atitudes mais eficazes, até agora, são as mesmas que eram recomendadas na idade média: distanciamento social, prevenção de contactos e confinamento. A pandemia vulnerabiliza a humanidade no seu todo, justifica restrições massivas à circulação e às liberdades, precipita uma crise económica de proporções ainda desconhecidas e ninguém sabe o que acontecerá no futuro, aos sobreviventes e à possibilidade de recirculação do vírus. É tempo de refletirmos. A organização da sociedade vai mudar e nós seremos protagonistas.

Window of Opportunity for Immunodulatory Therapy in COVID-19

A pandemia pela COVID-19 é um problema grave de Saúde Publica mundial, para a qual não existe tratamento eficaz. Cerca de 20%-33% dos doentes são admitidos em Unidades de Cuidados Intensivos e nestes a taxa de mortalidade é superior a 25%. A morte causada diretamente pela COVID-19 resulta, na maioria dos casos, de uma resposta hiperinflamatória inapropriada à infeção, com libertação de citocinas. Pelo paralelismo com outras doenças inflamatórias imunomediadas, os autores fizeram uma revisão crítica dos mecanismos fisiopatológicos envolvidos e potenciais situações onde a utilização de imunomoduladores, numa estreita janela de oportunidade, pode vir a mudar o prognóstico de alguns doentes.

The COVID-19 pandemic is a serious public health problem worldwide, for which there is no effective therapies. About 20%-33% of patients are admitted to intensive care units and in these the mortality rate is higher than 25%. Death caused directly by the COVID-19 results, in most cases, from an inappropriate hyperinflammatory response to the infection, with the release of cytokines. Due to the parallelism with other immune-mediated inflammatory diseases, the authors made a critical review of the pathophysiological mechanisms involved and potential situations where the use of immunomodulators, in a narrow window of opportunity, may change the prognosis of some patients.
