Solitary eosinophilic granuloma of bone is among the benign lesions of osseous location and usually run an indolent course. First described in 1940 by Lichtenstein and Jaffe, this entity has no determined cause and its association with other syndromes was suggested. Its usual location is the skull, femur and spine and can appear as single or multiple lesions. It has often a good prognosis but treatment options still lack consensus. The authors present a rare case of a 36-year-old man who showed an eosinophilic granuloma with an atypical location - the clavicle - complicated with frature. Operative treatment was performed with tumor mass excision and stabilization with autologous bone graft and plate. Excellent functional outcome was achieved and maintained through a 3 year follow-up. To our knowledge, there is no data in literature reporting pathological frature on presentation of a solitary eosinophilic granuloma.
