Recently an association between long term therapy with biphosphonates and the appearance of atypical femoral fractures have been described. The authores present a clinical case of a 70 years old woman, taking biphosphonates for more than 5 years who had a minor fall in June 2008 that resulted in a right subtrocantheric fracture. At that time, after surgical procedure, it was chosen to maintain usual therapy. New episode of fall at February 2011 that resulted left femoral diaphysis fracture. At this time and after surgical procedure, it was chosen to suspend biphosphonates therapy and initiate alternative therapy. This clinical case serves to alert for an individual fracture risk assessment when considering long term biphosphonate therapy, It may be appropriate to consider a drug holiday particularly as the cumulative duration of bisphosphonate therapy surpasses 5 years.
