Despite maintaining its guidelines in the treatment of rhizarthrosis, trapezium-metacarpal arthrodesis is associated with loss of mobility and load transfer to the neighboring joints. The aim of this paper is to present the results of three patients with trapezium-metacarpal arthrodesis who, for having poor functional outcome, underwent desarthrodesis and trapezium-metacarpal arthroplasty with "ball-and-socket" prosthesis. Performed clinical and radiological evaluation of 3 patients with a mean follow-up of 18 months. There was a frank decrease of pain and improvement in daily living activities evaluated, as well as a gain of mobility and strength of the operated thumb, with a early return to daily living activities and employment. The total trapezium-metacarpal arthroplasty for arthrodesis surgery revision of this joint is a valid and effective treatment option with good functional results. Proper selection of candidates for this procedure is a key step in achieving these results.
