Proximal Row Carpectomy is a common technique employed in the treatment of radiocarpal arthritis. This technique has been traditionally contraindicated in cases where the capitate has focal condral lesions. In knee surgery, mosaicoplasty has been used to treat focal condral lesions and this concept has the potential to be applied in wrist surgery at the same conditions permitting the broadening of proximal row carpectomy indications. We present a clinical case of a 66 year old man with a symptomatic SNAC wrist with focal condral lesion of the capitate, classified as grade IV in Outerbridge classification, in which we utilized the technique of proximal row capectomy associated with resurfacing of the capitate lesion with autogenous osteochondral graft from excised lunate. After 17 months of follow-up the patient shows a good functional result with maintenance of mobility and return of daily activities. We describe and discuss the indications to utilization of the technique.
