Coronal distal humerus fractures may involve the capitellum, trochlea or both. These fractures are rare in adults and even rarer in children. We present the case of a 13 year-old girl with a capitelar-trochlear shear fracture, classified as McKee type 4. She underwent an open reduction and internal fixation with 2 posterior to anterior screws. At the time of 3 months follow-up, the fracture healed and the patient presented with no complaints and good mobility of the elbow. Careful evaluation of the fracture pattern and identification of concomitant injuries is essential and should always be performed. Anatomic reduction associated with a stable internal fixation, allowing early mobilization is crucial to obtain the best results.
