Martin-Gruber anastomosis is a confounding factor in the clinical diagnosis of carpal tunnel syndrome to which clinicians are not usually alert. Its association with congenital proximal radio-ulnar synostosis is not yet described in the indexed literature, possibly due to the rarity of both conditions. We present a case of a patient with a clinical picture of bilateral carpal tunnel syndrome, associated with Martin-Gruber anastomosis and proximal bilateral radioulnar synostosis, who presented a diagnostic and therapeutic challenge.
