Recurrent patellar dislocations is often associated with preexisting anatomic factors. Trochlear dysplasia is known to be a major risk factor for patellofemoral instability. Indication for trochleoplasty is recurrent patellar dislocation with highgrade (type B and D) trochlear dysplasia. The authors describe a case of a patient with right knee pain and recurrent episodes of patellar dislocation. Clinically patellofemoral instability, imaging of trochlear dysplasia type B. Patient was submitted to trochleoplasty according to Dejour technique associated with medial patellofemoral ligament reconstruction. Trochleoplasty is technically demanding, usually combined with medial patellofemoral ligament reconstruction that was found to be very successful resulting in improvement in knee function like in this case.
