Objective: Recurrent dislocation of the extensor carpi ulnaris tendon is a rare cause of ulnar wrist pain, caused by breakage or attenuation of fibrotendinous sheath that surrounds it. Our goal is to present a case of a patient with a traumatic dislocation of the extensor carpi ulnaris tendon surgically treated. Description: Female, 37 years old, referred to an hand surgery consultation for cubital pain in her right wrist with sudden onset after pulling a cord of a chainsaw. Objectively she had a painful rebound of the extensor carpi ulnaris tendon and a dorsal subluxation of the distal radio-ulnar joint. The patient underwent surgical treatment proceeding to the suture of the cubital margin of the extensor carpi ulnaris sheath with adequate stabilization of the triangular fibrocartilage complex and the extensor carpi ulnaris in its groove. At the time of the last follow-up the patient had great mobility without instability or new episodes of dislocation. Commentaries: The tension of the extensor carpi ulnaris tendon in his fibrous tunnel is an important stabilizing factor for the distal radio-ulnar joint. Despite the satisfactory outcome, more studies are needed to establish whether this condition should be treated conservatively in acute cases.
