^lpt^aA tuberculose extrapulmonar envolvendo a mão é uma entidade clinica rara e o seu diagnóstico difícil. Pode manifestar-se a nível cutâneo, subcutâneo, tecido sinovial, artrite ou mesmo osteomielite, mas é pelo tecido tenosinovial que a bactéria tem predileção e mais pelos tendões flexores que extensores. A nível do punho pode originar compressão no Nervo Mediano que se manifesta como um vulgar síndrome do túnel do carpo mas de diferente diagnóstico e tratamento. É comum os doentes omitirem ou desconhecerem os seus próprios antecedentes pessoais, pelo que é necessário que o clínico tenha a patologia presente e um elevado índice de suspeita, afim de a poder diagnosticar e tratar corretamente. Descrevemos um caso clinico de um doente com síndrome do túnel do carpo, provocado por sinovite tuberculosa, enunciamos também alguns sinais de alarme, com o intuito de alertar sobre o seu correto diagnóstico e tratamento.^len^aExtrapulmonary tuberculosis affecting the hand is a rare clinical condition and its diagnosis isn’t easy. It can affect the skin, the subcutaneous and synovial tissue, articulations or even bones, but it’s the synovial tissue the preferred for the bacteria, and the flexor tendons are chosen over the extensors. In the wrist it can simulate a carpal tunnel syndrome but with a different etiology and treatment’s approach. It is known that patients often lie or aren’t aware of its owns past history, so it is very important the physicians suspicion in the presence of any sign of alarm in order the condition to be recognized and properly treated. The authors describe a clinical report of a carpal tunnel syndrome caused by a flexor synovial tuberculosis infection and the signs that should raise suspicion for the proper diagnosis of the condition.
