Introduction: The patellar height may be important in determining function after total knee arthroplasty. Patella baja after a total Knee arthroplasty is an indicator of poor prognosis, responsible for a considerable percentage of revisions. Total knee arthroplasties with sacrifice of the posterior cruciate ligament may eventually lead to a further lowering of the patella. Objective: To evaluate differences in alteration of patellar height after total knee arthroplasties with and without preservation of the posterior cruciate ligament. Method: A retrospective analysis was made of pre- and post-operative X-rays of 229 patients submitted to total knee arthroplasties. The authors used the modified plateau-patella angle method. Statistical analysis was performed using IBM SPSS software. Results: The mean variation of patellar height was -3.6 . In the group with preservation of posterior cruciate ligament was -3.5, and in group without preservation of the posterior cruciate ligament was -3.9 (p = 0.55) Discussion: A tendency towards decreased patellar height was seen in both groups. Although that decrease was been more evident in the group in which the posterior cruciate ligament was sacrifice, the difference wasn’t statistically significant. Conclusions: There were no statistically significant differences depending on the preservation or sacrifice of the posterior cruciate ligament.
