Rotator cuff injuries are rare in adolescents, accounting for less than 1% of shoulder pathology in patients under 20 years of age. At this age, the bony equivalents like avulsion of greater and lesser tuberosities are much more common. The objective of this paper is to describe a rare injury pattern: combination of bony avulsion of the greater tuberosity associated with a complete tear of the infraspinatus tendon, in an adolescent athlete. This lesion, involving the insertion of the rotator cuff from anterior to posterior, can be described as an osteotendinous injury of the humeral head. The patient was submitted to surgical treatment, using trans deltoid approach, open reduction and fixation. We emphasize the degree of bone maturity by itself doesn’t exclude the possibility of a rotator cuff injury. Several authors recommend the identification of all possible lesions in adult populations. The surgeon may choose more advance imaging techniques as MRI and should be prepared to address this type of lesion and provide proper treatment.
