Herein we present the clinical case of a 13 year-old female patient with left inguinal pain ongoing for 3 years, without any abnormalities in clinical examination. The imaging studies identified a radiolucent lesion in the left femoral proximal metaphysis and neck, with an incomplete fracture. Due to persistent pain, lesion characteristics and fracture location with an unknown aetiology, a surgical procedure was performed, which simultaneously allowed the definitive diagnosis of fibrous dysplasia. Patient’s post-op follow up was uneventful with good clinical result despite the imagiologic tumoral relapse. This clinical case emphasises the difficulties to diagnose and treat fibrous dysplasia in pediatric patients. Simultaneously, highlights the role for surgery, on which a good indication and technique will allow a good result.
