Introdução:
A nevralgia do trigémio consiste numa dor muito intensa, com início súbito e de curta duração, espoletada por estímulos inócuos. No entanto, pode manifestar-se inicialmente de forma atípica tornando o seu diagnóstico mais difícil.
Descrição do caso:
Relata-se um caso de uma mulher de 77 anos de idade com dor tipo moinha no maxilar superior direito, que motivou múltiplos contactos com os cuidados de saúde, tendo sido submetida a vários ciclos de antibioterapia e à extração de um dente, por suspeita inicial de patologia dentária. Posteriormente, a dor evoluiu para um padrão típico de nevralgia do trigémio, pelo que se assumiu este diagnóstico. Iniciou-se terapêutica para controlo da dor com carbamazepina e fez-se o estudo de causas secundárias, no qual foi identificado um meningioma no trajeto do nervo trigémio direito.
Comentário:
As manifestações atípicas da nevralgia do trigémio podem levar a um atraso no diagnóstico desta patologia pelo que é importante ter um elevado nível de suspeição. A terapêutica sintomática deve ser iniciada aquando do diagnóstico e deve ser considerado um estudo de causas secundárias.
Introduction:
Trigeminal neuralgia is a chronic pain disorder characterized by a sudden onset of high-intensity pain triggered by some innocuous stimuli. However, in some cases, it may have atypical manifestations that make diagnosis more difficult.
Case description:
This case reports a 77-year-old woman with dull pain in the upper right jaw that motivated multiple visits to health institutions. The patient underwent several courses of antibiotics and a tooth extraction due to suspicion of dental pathology. Subsequently, the pain evolved to a typical pattern of trigeminal neuralgia and this diagnosis was established. Pain management with carbamazepine was started and the study of secondary causes was initiated resulting in the identification of a meningioma in the right trigeminal path.
Comment:
The atypical manifestations of trigeminal neuralgia can lead to a misdiagnosis of this pathology and for this reason a high clinical suspicion is important. The symptomatic therapy should be initiated at the time of the diagnosis and an investigation of secondary causes should be performed.
