The selection of a contraceptive method in the adolescents with systemic lupus erythematosus (SLE) has been controversial, namely with respect to combined hormonal contraceptives for the risk of thrombosis and possible impact in the disease activity. Recent reviews suggest that patients with SLE may use the majority of the contraceptive methods, including the hormonal ones. The combined hormonal contraceptives are a good option in lupic adolescents with negative antiphospholipid antibodies, without other risk factors for thrombosis and inactive disease. Progestogen-only contraceptives are an option in the patients with higher risk of thrombosis or active SLE. Barrier methods are globally recommended, emergency contraception has no contraindication in the adolescents with SLE; intrauterine device is not actually contraindicated being considered a safe option, specially the one containing levonorgestrel.
