Abstract

Recognizing the remarkable advances in Gastroenterology, the incapacity of the general gastroenterologist to dominate all areas of knowledge and the needs of the country, lead to a new design in the National Training Program with the inclusion of the subspecialisation in Hepatology. The hepatologist should acquire a level of specific knowledge and skills which allow the diagnosis and management of most complex liver diseases and the integration in liver transplant teams. After completing the training program of at least 18 months in a certified centre, trainee must be assessed in a final exam. After integration in a gastroenterology unit, his mission should be to exclusively manage liver disease patients and to exercise the role of a consultant. Due the extension and specificity of many areas of Gastroenterology, I propose a new training program which would maintain 5 years duration allowing fellows to shift to specific Hepatology training in the final year. This proposal does not prolong too much the duration of training, preserves the unity of Gastroenterology and Hepatology, permits greater flexibility in the curriculum and allows the development of specific competencies according to the fellows interests and motivation.
