Introduction: The adoption of systematization of nursing care is an important tool for the organization of practice, however, there are difficulties in performing it in Primary Health Care (PHC). Objective: Analyze how nurses perform the systematization of nursing care (SNC) in the Family Health Strategy. Methods: This is a qualitative, exploratory and descriptive research, the data were collected through semi-structured interviews, with an intentional sample of 19 nurses. Data analysis was carried out through Content Analysis, thematic modality. Results: There is a diversity of references in the perform of the SNC, with no definition of guidelines by the municipal management. With the implementation of the e-SUS (a strategy to restructure Primary Care information at the national level, developed by the Brazilian national health system) and the electronic medical record, new challenges arose, requiring the training of professionals to understand the new logic of carrying out and registering the care provided. Conclusions: The thematic analysis contributed to the identification of the challenges and strategies for the implementation of the CNS in PHC.
