Retinal angiography with sodium fluorescein is a safe diagnostic method with an extremely low mortality rate. However, it is also the most common cause of outpatient emergencies in ophthalmology clinics due to the risk of unpredictable adverse reactions. The prevalence of fluorescein allergy is not known. We present the case of a man with a history of several retinal angiographies. During his last procedure, he developped acute urticaria. Intradermal skin tests were positive on the 1/10 dilution. Most adverse reactions to fluorescein are due to non IgE-mediated mechanisms. Fluorescein allergy can be confirmed by skin testing. Fluorescein angiography presents potentially fatal risks, such as anaphylaxis, which should be rapidly identified and effectively treated
