Allergen immunotherapy consists in the administration of allergen extracts with the aim of inducing tolerance to allergens responsible for the allergic reactions, as observed in hymenoptera venom allergy. Venom immunotherapy is indicated in patients with insect sting allergy with severe or moderate systemic reactions and IgE-mediated mechanism. The authors present the case of a 40 year-old patient submitted to bee venom immunotherapy with an ultra-rush protocol that developed large local reactions at the vaccine injection site. During the maintenance phase of the vaccine there was gradual worsening of the large local reactions with erythema, edema and wheal. Following clinical and etiologic investigation, cold-induced urticaria was diagnosed and controlled after appropriate treatment and eviction of ice application after vaccine administration. The authors report the case drawing the attention to the indications of venom immunotherapy and that, besides the local and systemic reactions to immunotherapy already known in the literature, there may be less common side effects which may be related to other pathologies not previously diagnosed
