Strongyloides stercoralis generally causes asymptomatic infection or mild gastrointestinal and pulmonary symptoms. In immunocompromised hosts the overwhelming accelerated autoinfective cycle can cause a life-threatening illness with multiorganic failure due to a massive larval dissemination. The authors describe a case of an HIV-infected male patient with a disseminated strongyloidiasis complicated by progressive paralytic ileus. treatment with albendazol and ivermectin administered by oral and rectal routes was unsuccessful because of gastrointestinal intolerance, thus evidencing the need of new therapeutic options.
