Inflammatory bowel disease (IBD) is a chronic condition that increases the risk of development of colorectal cancer (CRC). Chronic inflammation is a hallmark of IBD and leads to oxidative stress, which causes DNA damage directly or by lipid peroxidation. Further, oxidative stress impairs DNA repair mechanisms and the antioxidant response. Altogether these mechanisms induce a mutant phenotype, which may contribute to the carcinogenesis associated with IBD. In the future, the identification of these DNA lesions can be useful to the screening, diagnosis, treatment and follow-up of patients with IBD, namely those with high risk of CRC development. This paper reviews the most important mechanisms by which the oxidative stress damages the DNA in patients with IBD.
