Cytomegalovirus colitis in the immunocompetent host is rare but there is a higher morbidity and mortality in this group of patients. The authors describe a case of a 75-year-old woman patient admitted for bloody diarrhea over four weeks with a past history of diabetes, cardiac and chronic renal failure. Physical exam revealed dehydration, skin pallor, hypotension with signs of cardiac failure. Laboratory tests revealed anemia; chronic renal failure with acidosis and increased reactive C-protein. After stabilization Colonoscopy was performed which showed: swollen mucosa and ulcers of different sizes, with distribution target, in the entire colon. Histopathology was compatible with cytomegalovirus colitis. The patient was treated with ganciclovir with clinical, endoscopic and histological improvement.
