Tuberculosis is an endemic disease with increasing incidence in recent years. Among the extrapulmonary forms is otitis media tuberculosis (TOM). With an incidence of 0.04 to 0.09%, TOM is rare as a primary focus. The authors present a case of woman with recurrent otitis media, bilateral hypoacusis and left facial paralysis. she underwent radical tympanomastoidectomy which histology showed a granulomatous process. A Computed Tomography scan of the chest showed pulmonary cavities and PCR (polymerase chain reaction) of the ear exudate and bronchial lavage identified the presence of Mycobacterium tuberculosis. The patient was started on tuberculostatic medication, with a good clinical and radiological response. The authors highlight the importance of early diagnosis this disease, which, although rare, has considerable morbidity if not treated early.
