Introduction: Thymus cysts are lesions not frequently seen on our daily practice. They are mainly diagnosed during adolescence, but can be identified in any age group. These lesions are usually assymptomatic. The diagnosis is usually made by histological examination because imaging studies not always allow the differential diagnosis with more common lesions such as cystic hygroma. Surgery is the treatment of choice and prognosis is excellent if the lesion is completely removed. Case report: The authors describe a case of a male sex adolescent with 12 years old presenting with a cervical mass varying in size with Valsalva maneuver.
