^lpt^aA não compactação isolada do ventrículo esquerdo (NCIVE) é uma causa rara de miocardiopatia dilatada, verificando-se dilatação do ventrículo esquerdo, com exuberante padrão trabecular. Uma das manifestações são os fenómenos tromboembólicos, com prevalência entre 21% e 38%. Descrevemos o caso de uma mulher de 37 anos, cujo acidente vascular cerebral (AVC) isquémico foi a forma de apresentação da NCIVE. Realizou trombólise com reversão total dos défices. No estudo efectuado foi detectada NCIVE, além de foramen ovale patente, admitindo-se cardioembolismo como causa do AVC, com início de hipocoagulação oral. Realçamos a NCIVE como etiologia pouco comum de AVC e a importância do seu estudo etiológico no jovem, ao influenciar a decisão terapêutica e o prognóstico a longo prazo.^len^aIsolated left ventricular noncompaction (ILVNC) is a rare cause of dilated cardiomyopathy, with dilated left ventricle, and exuberant trabecular pattern. One of the manifestations is thromboembolic phenomena, with prevalence of 21% - 38%. We describe the case of a 37-year-old woman whose ischemic stroke was the presentation form of isolated left ventricular noncompaction. She underwent thrombolysis with complete reversal of the deficits. ILVNC was detected in the study, in addition to patent foramen ovale. The cardioembolism was admitted as the cause of the stroke, leading to the decision of oral hypocoagulation. We emphasize ILVNC as an uncommon etiology of stroke and the importance of the etiological study of stroke in young patients, because it influences the therapeutic decision and the prognosis.
