The authors describe a case of a 61 year-old woman, diagnosed with stage IV breast cancer. At the time, she refused therapy. She is referred to the emergency department a year after, in mutism, and the computed tomography revealed a brain injury, taken as a new metastasis. In its clinical file, it was found an advance directive will. This is a formal document that spells out which medical care the person want to be given at the end of life. Here is presented the clinical case, the advance directives of will and a reflection about how these should be integrated into the clinical practice.
