Choledochal cysts are diagnosed in adulthood in 20-30% of cases, with several clinical pictures. The authors present the case of a 46 years old male with several cholangitis episodes over a four years period, accompanied by epigastric pain and weight loss over the last two. An endoscopic retrograde cholangiopancreatography showed a cyst communicating with the common bile duct, with a stenosis in the transitional zone. Surgery was performed and the pathological exam confirmed the diagnosis of a choledochal cyst. The importance of this diagnosis is related to its complications, including cholangitis, pancreatitis and cholangiocarcinoma. Congenital disorders of the biliary tree must be considered in the differential diagnosis, even in adults.
