Introduction: Autoimmune hepatitis occurs more frequently in young women; however, it is increasingly diagnosed in advanced age. This is due to the increase in average life expectancy and a greater awareness of this diagnosis. Case Report: A 81 year-old woman was admitted with jaundice. Laboratory studies showed a pattern of “acute hepatitis.” Further investigations revealed auto-immune hepatitis, with evidence of liver cirrhosis. She started immunosuppressive therapy with prednisone and azathioprine, with a rapid response. Discussion: The differential diagnosis of acute hepatitis in the elderly is discussed, and some specific features of autoimmune hepatitis in this age group. Conclusion: The diagnosis of autoimmune hepatitis should be considered at any age, since appropriate therapy significantly reduces mortality associated with this entity.
