Background:

Although it is a rare condition, the Pseudo-Meigs’ syndrome should be considered as a differential diagnosis in patients presenting the triad: ovarian mass, ascites, and raised serum levels of CA-125.

Case description:

The authors report the case of a 67-year-old woman, complaining of fatigue, edema, abdominal discomfort, and weight gain. As relevant background, total thyroidectomy stands out. On physical examination, just abdominal distention is highlighted. The analytical and imaging study indicates a malignant neoplasm of the ovary with ascites. She underwent a total hysterectomy and bilateral adnexectomy. After the histopathological study, the diagnosis of Pseudo-Meigs’ syndrome with Struma ovarii was established.

Comments:

Pseudo-Meigs’ syndrome is characterized by ascites and/or pleural effusion, an ovarian tumor (other than fibroma), and elevation of CA-125 serum levels. The Struma ovarii is a variant of teratoma, which consists mostly of thyroid tissue. Although a rare condition, it must be considered, since extensive surgery can be avoided in the presence of this diagnosis.
