Introduction: The horizontalization of care is a form of organization of work in health, in which there is a multidisciplinary team of reference that works daily in the service, as opposed to the form of organization of work in which professionals have a workload distributed per shift. Objectives: Understanding the work process related to the duties of the horizontal nurse in the multidisciplinary team. Methods: This is a descriptive, exploratory qualitative study, carried out with 48 professionals from a public hospital. Data collection took place from October 10 to November 30, 2022. Data were analyzed using Bardin’s content analysis: pre-analysis, material exploration and treatment of results, inference and interpretation. Results: It was possible to find three nuclei of meaning that were divided into three categories: 1- Lack of routine work systematization; 2- Work process in the team's perception; and 3- Learning needs. Conclusions: It was verified that the role of the horizontal nurse is not properly clarified, therefore requiring the elaboration of institutional protocols that present the activities to be developed by the horizontal nurse and by the on-duty nurse. In order to align conducts in the hospital scenario, it is suggested extensive training of the multidisciplinary team with a horizontal care approach, recommended by the National Policy for Hospital Care.
