Atraumatic sulcal subarachnoid hemorrhage is a rare entity. Various aetiologies have been postulated and include carotid atherosclerotic disease among others. We illustrate the case of a man admitted to the emergency department with paroxysmal headache and dysarthria. Imaging studies disclosed sulcal subarachnoid hemorrhage and right extracranial internal carotid artery occlusion. This entity represents an important subtype of subarachnoid hemorrhage and the main challenge relies in the underlying cause diagnosis.
