Introduction: Hypersensitivity to proton pump inhibitors (PPIs) is rare and cross-reactivity between PPIs must be taken into account. The authors document a case report of delayed hypersensitivity to all PPIs available in Portugal. Case report: 39 year-old woman referred due to drug allergy suspicion, with past history of allergic rhinitis and gastro-esophageal reflux. In 2007 due to a flu-like syndrome she was administered with aspirin and omeprazole intravenously, and generalized itchy rash with lips angioedema was observed 3h later; in 2008 in a similar clinical picture, after oral intake of ibuprofen and omeprazole a reproducible reaction after 6-8h was observed. In 2010, due to gastric complaints she started on omeprazole (40mg/day) but suffered from a generalized rash 6h after the first administration. The sequential intake of lansoprazole, pantoprazole and esomeprazole originated a reproducible reaction. Rabeprazole originated a bullous exanthema in the upper limbs. Our allergological investigation has shown negative prick tests, but intradermal tests were positive (with delayed lecture at 10h) for the 5 PPIs. Conclusion: This is the first reported case of delayed PPI hypersensitivity to all PPIs
