Severe asthma affects about 5-10% of asthmatic patients and is an important cause of school and work absenteeism, poor quality of life and high health costs. Clusters are recognized by demographic, clinical and/or pathophysiological characteristics called “asthma phenotypes”. Regarding the inflammatory phenotypes, we can currently identify 3 groups in asthma: eosinophilic asthma, non-eosinophilic asthma with predominance of neutrophils and paucigranulocytic asthma. The aim of this article is to review the literature on bronchial asthma and the usefulness of macrolide therapy in patients with severe asthma. Macrolide therapy appears to have demonstrated some efficacy in steroid resistance (steroid dependence) phenotypes, such as non-eosinophilic asthma
