We present the case of a eight years old patient with a long gap esophageal atresia. During the first months of life she had been in exclusive enteric nutrition trough a gastric tube waiting for esophageal growth. At eight months she was submitted to surgical esophageal anastomosis under significant tension and developed important gastroesophageal reflux. In spite of permanent treatment with proton pump inhibitors a significant esophageal stricture appeared in the anastomotic region, which did not resolve with dilations and needed a new surgery. After this complicated course, patient’s growth was normal, but she had some episodes of dysphagia with spontaneous resolution. In the esophageal endoscopic control by the age of eight years, we observed a deforming scar with a pseudodiverticular aspect without inflammation. Usually, esophageal growth minimizes the diverticular aspect and the dysphagia in these patients, who should be submitted to periodic endoscopic control during life.
