Hematuria is a common condition that can range from an incidental finding to a significant symptom requiring immediate treatment. It can be associated with a variety of conditions, but once urinary tract and renal disease have been excluded, vascular causes, including nutcracker syndrome (NCS), should be considered. NCS is a diagnosis of exclusion, and Doppler ultrasound is recommended as first-line investigation. Hematuria associated with NCS is an underestimated entity, which the authors aim to raise awareness of with this case.
