With the aim of reviewing the most recent recommendations regarding the approach of the patient in acute respiratory failure in an emergency context, namely those that are the difficulties and doubts we can find, we present the ABCDE methodology, the approach to the patient with acute asthma or acute obstructive pulmonary disease, and also the patient with acute heart failure, and the differential diagnosis between them. Therefore it is discussed the clinical approach to the emergent patient in internal medicine and the most frequent etiologies of acute respiratory failure in this context, resorting to clinical cases and the doubts that frequently arise in this environment, conditioned both by the inherent stress and the complexity of the unstable patient.
