At present, respiratory failure (RF) is commonly defined as a laboratory abnormality, evident only after performing arterial blood gas analysis (ABG), instead of being recognized as a clinical syndrome with characteristic symptoms and signs, different stages and severity levels, and with aetiology dependent physiopathological features. This often complicates its clinical diagnosis, especially in early stages, hampers its study and understanding and prevents appropriate treatment to be given in due time, in numerous situations. Further-more, it hinders clinical research for a better understanding of the syndrome and its treatment, in different homogeneous groups of patients with RF. Clinical practice demonstrates that we have an incomplete knowledge of RF, its diagnostic approach, management and treatment. RF directly impacts patient’s quality of life and prognosis. It should be considered a nosologic entity, with unique clinical, physiopathological and dynamic characteristics, that demands a dedicated investigation aiming to be better understood and treated with the goal of changing its course and improve clinical outcomes.
