Acute esophageal necrosis is a rare entity endoscopically caracterized by a black esophageal mucosa. It has been associated with caustic ingestion, medication, local infection, and, more frequently, with ischemia secondary to haemodynamic instability. More recently there have been some cases related to acute and chronic alcohol ingestion, and related diseases, allowing the possibility of a relation between the two entities. The authors describe a case of black esophagus in a male patient admitted after an episode of hematemeses followed by hypovolemic shock and aspiration pneumonia due to acute alcoholic ingestion. The direct and indirect effect of alcohol on the esophageal mucosa and ischemia secondary to hypovolemic shock and pneumonia are the factors most probably involved in the pathogenesis of this case.
