INTRODUCTION: Endometriosis is the presence of ectopic endometrial tissue, and is diagnosed in childbearing women. It is more frequent in organs and structures within the pelvic cavity. Outside the pelvis, the digestive tract is the most common location. CLINICAL CASE: Woman, 40 years old, who was admitted to our urgency complaining of hematoquezias. She had history of ectopic pregnancy and bilateral salpingectomy. The endoscopic examination identified the small intestine as probable source of gastrointestinal bleeding. Exploratory laparoscopy allowed identification of ovarian endometriosis with invasion of the ileum. CONCLUSION: Gastrointestinal bleeding is an uncommon form of presentation of endometriosis. It is important to consider this clinical entity in the differential diagnosis of gastrointestinal bleeding of obscure origin, especially in young women.
