Drug induced acute pancreatitis is a rare entity, whose true incidence is unknown. Its diagnosis is often difficult to establish, requiring a high degree of suspicion and careful exclusion of other causes of pancreatitis. In the last decades a growing number of drugs have been implied, with varying weight of evidence, including several ACE inhibitors. The authors present the case of a patient with several recurrent episodes of acute pancreatitis, in a short period of time, whose aetiology was attributed to perindopril after an exhaustive study that excluded other causes, a positive rechallenge test and favourable evolution after removal of the drug.
