AIM: Describe the surgical procedure of transumbilical cholecystectomy using standard laparoscopic instruments. PATIENTS AND METHODS: The study enrolled 12 patients, with average age 42,75 and average body mass index 30,76, who underwent transumbilical laparoscopic cholecystectomy. A 10 mmtrocar (scope 30°), 3mm and 5mm trocars were introduced in umbilical scar. In order to help dissection of cystic duct and cystic artery, a 2mm trocar was introduced in right flank if necessary. If available, a 5mm (30° scope) optic was introduced in a 5 mm trocar at the moment of clipping the cystic duct and cystic artery, giving passage to the clipper by the 10 mmtrocar. RESULTS: Six patients underwent single porttransumbilical cholecystectomy. Five used 2 sites. In a morbid obese patient, it was necessary to convert into a conventional laparoscopic cholecystectomy (four sites) due to massive bleeding from cystic artery. All procedures were performed using standard laparoscopic instruments. There were no postoperative complications and all patients had excellent esthetic effect. CONCLUSION: Transumbilicalcholecystectomy using standard laparoscopic instruments represents a safe, feasible and possible surgical option.
