The significance of intraductal papillary mucinous neoplasms (IPMNs) of the pancreas has increased in the last decade. IPMNs originate on the pancreatic duct and can present a wide spectrum of cyto-architectural dysplasia with well established malignant potential. Anatomically, IPMNs are subdivided into lesions originating from the pancreatic main duct, branch ducts or both, leading to different prognosis. The diagnosis, characterization and therapeutic management of anatomically diverse IPMNs is therefore essential, however difficult and most often requiring a multidisciplinary approach. Here we present two case reports of IPMNs, one of the main duct and the other of the branch duct, with different therapeutic strategies.We reviewed the literature on this entity, with special atention to the therapeutic planning of these lesions by their characteristics.
