The aim of the study was to describe the systemization of nursing care for a liver transplantation patient, in line with the physiological mode of Roy’s adaptation theory. The case study was performed at a university hospital in Fortaleza/CE, Brazil, from October to December 2008. Semi-structured interviews, observation and physical examination were used. After assessing behaviors and stimuli, the following nursing diagnoses could be elaborated: ineffective breathing pattern, imbalanced nutrition - less than body requirements, constipation, impaired physical mobility, self-care deficit for bathing/hygiene, deficient fluid volume, ineffective protection, risk for infection, impaired skin integrity, impaired memory, ineffective sexuality pattern. The main nursing interventions for the identified diagnoses are: administration of analgesics, hydration and nutritional monitoring, injury care, protection against infection, hydration control, health education and active listening. Roy’s nursing process permitted the identification of stimuli that gave rise to positive or negative responses, in which the nurse should act as a mediator, with a view to improving care delivery to liver transplantation patients.
