Abstract

Issues related to hospital discharge planning have been addressed recently in order to identify the factors that affect it and determine its effectiveness, and, consequently, enable continuity of care, patients’ recovery and their reintegration into the community. The aim of the current descriptive and correlational research study was the translation and validation of the Portuguese version of the PREPAREDquestionnaire. The sample of patients was composed of 129 individuals who were admitted for surgery for more than three days, and have been discharged from hospital. The sample of caregivers was composed of 118 main caregivers. The Portuguese version of the PREPARED patient and caregiver questionnaires were made available by this study. In a community context, it represents a valid structure for information acquiring on the perception of the quality of the activities regarding the discharge planning procedure.
