Background: Liver transplantation is a therapeutic modality in situations of advanced liver disease whose success depends on how patients deal with a new and complex therapeutic regimen in their daily lives. Objectives: To develop a flowchart to support nursing decision-making on the therapeutic self-management of liver transplant patients. Methodology: Qualitative longitudinal study conducted between January and December 2013 in a transplantation center, using retrospective content analysis of nursing records and consensus meetings with nurses. Results: The flowchart included the following areas: medication regimen, dietary regimen, lifestyles, and complications. Nurses reached a 90% consensus on the specifications of each intervention included in these areas, with the purpose of achieving continuity of care. Conclusion: The systematization of information in a flowchart allows for a better identification of each patient's needs with a view to preparing discharge, and the continuity and quality of care.
