There are several widely used sets of criteria for the diagnosis of dementia. ICD-10 and DSM-IV are the usual criteria for the diagnosis of dementia, but they can differ in the number of subjects classified as having dementia. In this study we analyzed the agreement of the two sets of clinical diagnostic criteria. We examined 897 subjects with suspected dementia syndrome. The subjects were submitted to a neuropsychological assessment and were classified as demented based on DSM-IV and on ICD-10 or as non-demented. We found that 82,6% of the cases were classified as demented by the DSM-IV criteria while only 63,8% of subjects were given a diagnosis of dementia according ICD-10 criteria. The agreement on the two criteria was 61,4%. The different percentage of demented subjects, depending the criteria used, has significant implications for research and treatment.
