Spinal cord injury (SCI) is a disruptive life event that implies physical, psychological and social readjustment by the individual. This study aims to analyze the impact of post-injury psychosocial adaptation on the purpose in life and self-efficacy of inpatients and identify the mediating effect of the quality of life on the relation of these two psychological variables. The sample was collected at the “Centro de Medicina e Reabilitação da Região Centro - Rovisco Pais” and included 39 participants, with SCI diagnosis, which were applied the Purpose in Life Test, General Self-Efficacy Scale, Hospital Anxiety and Depression Scale, Therapeutic Adherence Scale, World Health Organization Quality of Life and Posttraumatic Growth Inventory. The results showed that depression and quality of life were predictors of purpose in life and self-efficacy, respectively. It was also found partial mediation effect on the relationship self-efficacy → purpose in life, verifying that the positive association between self-efficacy and quality of life improves purpose in life after SCI. Thus, it emphasizes the importance of strengthening the sense of efficacy in this population.
