Adherence to treatment of people with alcohol-related problems is an issue that concerns professionals in their care practice. Adherence, defined as participation in the therapeutic process, is a key element related to the quality and effectiveness of treatment programs. Several intrinsic and extrinsic factors related to adherence have been identified, but others require more attention. The aim of this work was to investigate the sociodemographic and clinical characteristics as well as the components of the therapeutic process that are associated with participation in treatment among 150 people with alcohol abuse/dependence who attended treatment at the Porto Alcohology Unit of the Northern Regional Health Administration, Portugal. Through the analysis of attendance records, the participants were characterized in terms of sociodemography and alcohol-related problems. We assessed comorbid depression and anxiety on admission to treatment through the Hospital Anxiety and Depression Scale. After one year, we assessed participation in treatment as well as the situation of alcohol intake and taking medication for alcohol problems. The results indicated that those who continued on treatment had less anxiety disorder. Continuity in treatment was significantly related to compliance with abstinence and with taking adjunctive medication. Treatment with adjunctive medication to psychotherapeutic support is an explanatory factor for adherence. This result has implications for clinical practice, highlighting the importance of ethical care for people with alcohol-related problems through the reinforcement of the psychotherapeutic component in an assistance model of continuous mental health care.
