Adherence to medication treatment is fundamental to the success of therapy. The objective was to develop an instrument for the evaluation of Medicinal Adherence in Diabetes Mellitus (AMDM) and to evaluate its psychometric properties. This is a methodological study of the development and evaluation of AMDM, an instrument for measuring a health-related event. The final version of AMDM consisted of 20 questions with two parts, the first called AMDM-I, with 12 questions and the second part, called AMDM-II, with 8 questions. Content validation and reliability estimates (Cronbach's alpha / weighted Kappa) were performed. The AMDM showed good content validity, Cronbach's alpha test = 0.71, the weighted Kappa ranged from 0.76 to 1.0, and the predictive validity obtained sensitivity = 73.2% and specificity = 63.3%. AMDM was considered to be easy and dynamic to apply, appropriate to the context in which it was applied. At AMDM, items related to the participants' behavior in relation to the control of their stock of medicines, compliance with medical prescription, and use of alcoholic beverages were considered; and difficulties in the acquisition of medicines, including through the Unified Health System (SUS), a health system in force in Brazil. Other existing instruments have not addressed these issues. It was concluded that the AMDM presented acceptable reliability and validity, in addition to good interpretability, showing itself capable of evaluating what is proposed.
