Although they are achievements obtained from the Federal Constitution of 1988, with the institutionalization of health as a duty of the State and a citizen's right, situations of exclusion from health services are still observed for some populations. The objective was to identify the main challenges faced by the transsexual population in accessing and using health services. This is a literature review, carried out in the months of February and March 2020, using the databases SCIELO, LILACS and MEDLINE, evaluating publications in Portuguese, English and Spanish, between the years 2016 and 2020. Due to the process of marginalization and discrimination in health services, the search for care and assistance in these services, by the transsexual population, is reduced, which implies the removal of this clientele, and contributes to them managing the “care” with their body. Such an attitude reduces the chance of health services to carry out preventive and educational actions with this population. Although several studies with the theme have been found, it is necessary to analyze that the majority deals with issues of the trans population related to the process of sexual reassignment surgery and HIV / AIDS, limiting or even excluding concepts and situations related to access to health services in general, and social recognition.
